 Serial casting involves putting a fiberglass cast across the joint that needs to be stretched. Each week the cast is removed to check the child's skin and the available motion. The cast is put back on, slowly stretching the muscles each week. This may take three to six weeks. After casting, children often wear a brace to maintain the optimal position of the joint. Depending on the diagnosis, therapy may be indicated to improve the function with the new joint position. Serial casting can be done for the ankles, knees, elbows, wrists, hands and fingers. Diagnoses which may benefit from serial casting are cerebral palsy, muscular dystrophy, toe walking, traumatic brain injury, or any diagnosis which causes increased stiffness (hypertonicity) causing contractures.
